Family CONNECT

‘Coordinated Outreach and Navigation for Needs-Driven Engagement, Care, and Treatment’

The Family CONNECT program provides long-term wraparound and intensive case management services to CCPS students and families by utilizing the Evidence Based Practice Family Centered Treatment (FCT) model. FCT is a trauma treatment model of in-home therapy focusing on holistic, family derived goals related to family functioning, preservation, permanency, and reunification. FCT identifies solutions for families faced with disruption or dissolution due to external and/or internal circumstances including child welfare, mental health, substance abuse, developmental disabilities and juvenile justice.

The Family CONNECT program addresses concerns and needs identified in the 2025 LMB Needs Assessment by:

  • Providing additional behavioral and mental health support services for students
  • Reducing wait times for behavioral health services
  • Strengthening coordination and collaboration between CCPS, CCYSB, and other community service providers
  • Eliminating transportation barriers by providing in-home therapy services to families

FCT focuses on utilization of the internal strengths and resources of each family. Goals are collaboratively developed, and families engage in experiential activities inclusive of cultural, generational, systemic, and trauma influences. FCT highlights family voice, uncovering inherent values and beliefs while restoring safety, belonging and connectedness.
