The Assertive Community Treatment (ACT) Team utilizes a community-based model that provides comprehensive, recovery-oriented, mental health/substance use treatment to individuals with severe and persistent mental illnesses. The ACT Team is comprised of a multi-disciplinary mental health and support staff. The Team’s primary function is to provide therapy, case management, vocational and support services to individuals for whom traditional outpatient mental health services were unsuccessful in meeting their needs. ACT interventions (e.g., treatment and rehabilitative services) take place in community locations. This includes the client’s residence, place of employment, and, at times, shelters, jails and hospitals.
